Please include either the full name, email, or order number associated with your product subscription. Include any other important details here.

(Optional) What caused you to cancel? (If you are canceling. This doesn't impact cancelation, we're just curious.)

Can't afford it right now/budgeting

Don't use it as much as expected

Don't like it/not for me

Don't need it anymore (got what I wanted)

Have too much product on-hand already

Lasts longer than expected

Need to change something

Subscribing to a different product/bundle instead

Other

Yes, subscribe me to your newsletter.

I understand that sub cancelation can not be undone. The price & shipping rate will not longer be locked in & could change in the future if I make a new subscription. (More than likely just the shipping rate since it changes overtime, due to carriers *out of our control.) I understand I can retain my rate if I pause my subscription (indefinitely) instead.
